Note: Effective July 1, 2007, the mandatory Texas Complaint Notice must include the Texas Department of Insurance website and e-mail address is paragraph 6 of the notice. These changes are the result of amendments to Title 28, Sec. 16.01 of the Texas Administrative Code adopted by the Commissioner of Insurance.

Paragraphs 2, 3, and 4 remain optional for surplus lines contracts.

Policies and binders filed with the Stamping Office on or after July 1, 2007 that include a Complaint Notice should reflect the additions.
